Can you tell us if your 20% asymptomatic positive rate. means
20% of all patients who have no symptoms are coming back positive ?
or that 20% of your positive patients are asymptomatic?

We are finding that 1/5 patients who present in labor (or for elective C/S or IOL) with no COVID symptoms are positive using a rapid COVID-19 test. We are then able to triage those labors to one section of L+D and postpartum, to limit providers and ensure appropriate use of PPE. We are not testing labor support people as we require everyone in the hospital (employees, patients, support) to wear a basic mask at all times.

Hi Brigid,

We just got the rapid turn-around, universal testing up and running on L&D this week. I have heard concerns about swabbing the antepartum patients on admission and then relying on that test for days to weeks later when they deliver. Are you repeating the test at any interval for negative, undelivered patients? Do you know if there's a concern for a false negative if they are recently exposed?

We are also struggling with these unknowns, I look forward to hearing from others...
We currently do not retest unless patient shows new symptoms, as we have a "no visitor" policy. That said, 25% of our patients presenting in labor with no symptoms are testing positive, which I would extrapolate to a potential rate of 25% of asymptomatic HCW, the only people with whom our antepartum patients are interacting...
Another thorny problem with rapid testing of our "elective" inductions and C/S is when and if to retest those who are positive who need to reschedule? Our goal in delaying is to save PPE and to avoid impacting on initial mother/baby bonding, but waiting for labor is not the right answer for many women.
Eager for input from others in "hot spots"!
